Q 1: How do I know if I must be tested?
A. If you attended a school in Arizona that is recognized by the US Dept of Education you are exempt from taking the exam. If you do not meet the requirements then the National Certification Board exam is required. www.ncbtmb.com or www.fsmtb.org

Q 2: I am looking to move to Arizona within the next four to five months and would like to know what steps I need to take to have my license transferred from the state I am in.
A. An applicant is eligible for reciprocal licensure if either of the following applies:
1. The applicant has been licensed continuously in another state for the last five years preceding the filing of the application with the board and the other state has standards for massage therapists that are substantially equivalent to those in this state in other words is a licensing state. OR
2. The applicant holds a current certification from the NCBTMB or MBLEX.
B. When an applicant submits an application for reciprocity, the applicant shall also submit a letter or other document acceptable to the board showing whether any jurisdiction that has previously certified or licensed the applicant has instituted disciplinary proceedings or has unresolved complaints pending against the applicant. If a disciplinary proceeding or an unresolved complaint is pending, the applicant shall not be licensed until the proceeding or the complaint has been resolved in the applicant's favor.

Q 3: I am going to massage school out of state; will I still be able to get a license in Arizona?
A. ARS 32-4228 (C) a school located in another state that offers a training program in massage therapy or bodywork therapy may make application for approval to the board of massage therapy. The board of massage therapy shall grant approval to a school located in another state if the school demonstrates that the school received approval from an agency similar to the state board for private postsecondary education and that the school meets requirements substantially equivalent to the requirements imposed by the state board for private postsecondary education.

Q 4: Will I have to get continuing education? How many and can I carry them over?
A. You will need twenty five hours; CE’s are required during your licensure period before you renew your license. They do not carry over to the next renewal date.

Q 5. I lost my license, can I get another one?
A. There is a $25.00 fee (payable in Money Order or Cashiers Check only) for replacing a lost or stolen license.

Q 6. If I already have my license, how do I change my name or address?
A. All name or address changes must be submitted in writing. Please print your full name, your old address, and your new address. For a name change you must include proof. (copy of marriage license, divorce papers, or official court documents.) look in forms

Q 7: Fingerprint service locations.
A. You can have your fingerprints done by any law enforcement agency that performs fingerprinting services. Please contact them for times and fees. Otherwise, you can go to anyone who roles fingerprints. It does not have to be done in Arizona, however you must use the fingerprint card we provide for you.

Q 8: Where can I get a Fingerprint card?
A. They are mailed with the application packets; however you can request that we mail you one if you download the application or you can come pick up one from the office. DO NOT BEND THE CARDS.

Q 9: Who does not have to be licensed under the massage therapy law?
A. Review the statutes for the description of a massage therapist, if you meet the definition you must be licensed, make sure you read all of the statute for who is not required to be licensed before you assume you’re exempt.
Refer to this statute for individuals who are exempt.

Q 10: Where can I find any proposed changes to the current massage regulations?
A. The Statutes and Rules are posted on our website www.massageboard.az.gov

Q 11: Is there a list of approved courses for CE’s
A. No, on the home page look for proposed rules including CE requirements. Scroll down to page 10 there are 3 rules that apply to CE’s. Please refer to rule R4-15-302 make sure your courses are from an approved provider

Q 12: I Have 50 hours of CE’s can they carry over to my next renewal date?
A. No, when a licensee renews a license, the licensee must submit to the board evidence of the successful completion of at least twenty-five hours of continuing education in the practice of massage therapy, as approved by the board, during the immediately preceding two years.

Q. 13: When do I turn in my CE’s?
A. At renewal time. You will be sent a renewal form 60 days prior to your renewal date. It is your responsibility to renew on time, whether or not you received the renewal notice.

Q. 14: Can I work as a massage therapist if the directory says my application is pending
A. You cannot work as a massage therapist until you are issued a licensed by the board.

Q. 15: How can I expedite my application process?
A. To make sure your application is processed in a timely fashion. Make sure application is filled out correctly and complete, have your documents sent asap, we will keep (transcripts, NCBTMB or the MBLEX etc) in a file for 1 year. We will match your documents to your application once it comes in.

Q. 16: Can I still be grandfathered ?
A. Not date ended for Provisional (grandfathering) as of 12-31-2005
